Medical Certificate ("Sick Note" or "Doctor's Note") Guideline
At Trent Student Health Services, a medical certificate, commonly referred to as a “medical note,” “sick note,” or “doctor’s note,” will only be issued in situations where an illness or injury results in a significant disruption of academic work.
In all cases the student should first communicate with their instructor about their absence and/or missed coursework. The medical certificate must be completed by the physician who personally examined the student at the time of their illness.

- A recovered illness and/or previously missed labs or classes
- Minor illnesses (e.g. common colds, flu-like symptoms, urinary tract infections, headaches, etc.)
- A recognized disability or chronic health concern (please see Student Accessibility Services for information about accommodations and appropriate documentation)
- Long term sleep/wake problems related to lifestyle
All requests for documentation to support an academic petition (i.e. retroactive withdrawal, incomplete standing, aegrotat standing) should click here to follow the student procedure for seeking supporting documentation.
