Report an incident using the Student Incident Report.
The term "incident" may refer to an occurrence arising from situation or tasks that resulted in, or had the potential to result in, injury, illness, damage to health or property, or fatalities. If you are unsure of certain details, indicate so and submit the report with as much information as you have available. Health and Safety will follow up afterwards, should more details be required.
All incidents should be reported, including those which:
- Result in personal injury
- Could reasonable happen to a worker or other member of the University community
- Occur to a student while on placement, co-op, volunteering, or providing any other service to the University
If you are unsure whether or not something should be reported, report it.
