NEW Workplace Injury, Illness, and Incident Reporting tools

Trent University encourages all members of the University community to promptly report any safety concerns or incidents that may arise. Prompt reporting assists in identifying, assessing, and controlling hazards to prevent further injury or property damage. 

Workers that experience or witness an injury, occupational illness, or other risk that causes or has the potential to cause an accident or damage to property should first report the event to their immediate supervisor. 

The supervisor and worker should collaboratively submit an incident report to the Health and Safety office. The Injury, Illness, and Incident reporting process has recently been updated to Microsoft Forms, providing a streamlined and efficient way to quickly submit reports while gathering all necessary information. Incident reporting forms can be accessed on the Report a Hazard/Incident page of the Health and Safety website. 

It is vital that reports be submitted as soon as possible after an incident to ensure quick follow-up and to meet requirements of the Ministry of Labour and the Workplace Safety Insurance Board. Incident reports should be submitted with as much information as is available - do not hesitate to submit a report even if some details remain unknown. The Health and Safety Office will assist and guide throughout the information gathering and incident investigation process. 

All incidents in the workplace need to be reported, including those which: 

  • Result in personal injury, 
  • Result in modified work or lost time in work, 
  • Require medical attention, 
  • Occur to any employee during the course of their work, or on any property where Trent-sanctioned activities occur, or
  • Where no injury or damage was sustained, but given a slight shift in time or position, damage or injury could have easily occurred (near miss). 

If you are unsure whether or not something should be reported, report it.

If you have any questions or concerns regarding injury, illness, and incident reporting, please discuss with your supervisor, contact healthandsafety@trentu.ca, or reach out to a member on the Joint Health and Safety Committee.
