Release of Information authorization
In accordance with the Freedom of Information and Protection of Privacy Act, Trent International (TI) Office cannot disclose your confidential personal information without your specific consent. If you anticipate that you may be unable to consult the office personally, and would like to assign responsibility for such to an agency, friend or relative, you must submit this authorization for release of personal information. It will stay in effect until you cancel it in writing.
