Dear Faculty/Staff/Student,

To ensure that we are continuously improving and responsive to you and the rest of the community, we need your help to participate in our first Annual IT User Satisfaction Survey. The survey is open to all faculty, staff, and students and depending on your role at the university and your responses, will take ten to fifteen minutes to complete.

You have until Monday August 19th at noon to complete this survey. Additionally, you must complete this survey and provide your profile information through this survey to be entered into the draw. The draw will be held on Wednesday August 21st to select a winner from the pool of completed surveys. The winner will be notified via e-mail with instructions on how and when to collect their prize.
