In order to ensure reasonable notice to professors for exam accommodations, students are encouraged to complete the pre-intake questionnaire and submit appropriate documentation prior to March 22. After this date, we cannot guarantee that an accommodation plan will be in place for final exams.
How to Register:
1. Obtain Documentation from a health professional who can diagnose and outline the functional limitations of the disability as these relate to the academic environment.
a. Documentation should be as recent as possible.
b. Please review Documentation Requirements prior to moving on to the next step.
2. Complete the Pre-Intake Questionnaire. This step will take approximately 10 minutes to complete. Please provide as many details as you feel comfortable sharing at this stage as this will assist us in creating a meaningful accommodation plan with you. Be sure to upload documentation before submitting the questionnaire. A SAS advisor (Corinn Phillips or Jamie Matin) will contact you to set up an intake appointment.
