with your full name and appointment date and time.

If it's your first appointment, print this form and complete it:   Student Health Service Medical Form and our Privacy and Confidentiality Agreement (2pgs).

If it's your first appointment related to your MENTAL HEALTH, please print and complete this form as well as the Medical Form above (2pgs).

For the Consent to Disclose Personal Health Information, print and complete this form (1pg).

If you are an International Student who is going for a test or treatment with another health care provider, you will need to print out the UHIP Claim Form (2pgs) to bring with you to your appointment for billing purposes.
