Asked Questions
Forms are available in PDF format. The PDF forms are form-fillable - they
can be filled out online, and then printed. Please note: You cannot save
a filled out form UNLESS you have the full version of Adobe Acrobat (not
Adobe Reader). These forms require Adobe Reader version 5.0 or above.
Versions 4 and lower can produce errors such as forms printing out all
black, or failure of the form to load properly. Upgrading your Adobe Reader to version 7.0 is easy and free. Simply click on the Get Adobe Reader
icon to install it on your computer.
